Prototype for Medical Application

I can't say much for obvious reasons.
I need a protoype made for a proof of concept.
Medical Application
Operation is Based on Mechanical operation
In principle, need to move a cable (housed in container) back and forth, with a turn of knob external to container, in steps of appx 1mm
Cheap, made from off shelf parts.
Small container 100x100x200 mm
Cheap, One time use, dispoable.
