PHP Format Change + SMTP Check

I need a PHP script with a form where input is changed to specific outputs and display it on page.

Another part is a SMTP checker in PHP, a form where SMTPs (multiple) can be added by given input and the script is checking, if the SMTP is working. Result displayed on page.
